Independent Contractors


Transformations contracts with qualified professionals for clinical and social services.


Independently contracted providers will agree to and sign a contract with the agency prior to the initiation of services. 

Independently contracted providers are not directed on how to provide their services.  Independent providers are considered professionals who will determine the diagnosis, treatment plan and interventions for the client.  Contracted providers practice freedom of choice in selection of clients.  At no time will the agency assign clients.  The agency does not set schedules or determine practice locations or vacation times.  The independent contracted provider is responsible for his or her own transportation, phone, printer, fax, office space or other service tools.  Providers are responsible to obtain their own supervision for licensing and professional educations and training.  Independently contracted providers may terminate with a  client based on the provider’s clinical decision.  

Independently contracted providers shall submit invoices for their services along with documentation describing the service.  Invoices and documentation details are governed by sources external to the agency.
